What paperwork do I need to fill out?

Before your consultation we will need to have you sign a consent form to be evaluated/treated for the consult, as well as your complete medical/surgical history.

Do I need goal photos?

Yes! Please bring 4-5 that you like and 2-3 that you don’t to your consult. It will be nearly impossibly to finalize a surgery plan if you don’t show us what your goals are.

Do you accept insurance?

We are out of network with insurance companies. If you would still like to have your surgery with our amazing team, we will give you the information you need to submit to your insurance company yourself afterwards.

Do you have a consultation fee?

Yes, our consultation fee reserves an hour of the plastic surgeon's time and is applied to your surgery when scheduled.

Do I need to wear anything special?

No, just wear something comfortable, and easy to change for when we get photos.
